- social security number (but not required)
- driver's license
- birth certificate
- *Service address or meter number
- *Effective Date
- *Phone number
- *Mailing address
- *Credit reference or security deposit (based on a credit check)
- Email address
Stopping Service with Western
Please provide:
- *Account Number, Meter Number, or Address
- *Forwarding Address
- *Effective Date
- Whether the meter is being disconnected or transferred to someone else
*Required
